M&M Health Services, is a Home Community Based Services Waiver program incorporated in Washington DC, providing services to people with intellectually and other pervasive developmental disabilites in the privacy of their homes, supervised facilities (apartments) and other community residential options.

Our Mission

The mission of M&M Health Services, is to provide people with developmental and intellectual disability opportunities to reach their highest potential by promoting self determination, person-centered outcome driven support, and develop natural support/relationships that connect to community resources ensuring an integrative and inclusive life experience.

Our Services

In-Home Supports
In Home Supports services are provided to people that reside in their natural homes. Services are driven by the persons’ ISP and are intended to provide.
  • Training and support in activities of daily living and independent living skills.
  • Assistance with developing the skills necessary to reduce or eliminate behavioral episodes by implementing a Behavioral Support Plan (BSP) or positive strategies;
  • Training on, and assistance in the monitoring of health, nutrition, and physical wellness.
  • Assistance in performing personal care, household, and homemaking tasks that are specific to the needs of the person, except that this may not comprise the entirety of the service;
Companion Services
Companion services provide non-medical assistance and supervision to support a person’s goals, desires, and needs as identified in the person’s Individual Support Plan (ISP), and reflected in his or her Person-Centered Thinking and Discovery tools. Goals may be related to the person’s safety, promotion of independence, community integration, and/or retirement.
Respite Care Services
Respite services provide relief to a person’s family or primary caregiver to enable them to participate in scheduled or unscheduled time away from the person, and to prevent gaps in the delivery of the person’s services. Respite Care Services may be provided in the natural home setting or out of home setting.
Supported Living
Supported living services are provided to persons enrolled in the Waiver who have limited informal supports and have an assessed need for assistance with acquisition, retention, or improvement in skills related to activities of daily living, and who require assistance with the development of social and adaptive skills that are necessary to enable the person to reside in the community and successfully participate in community activities based upon what is important to and for the person as documented in his or her Individual Support Plan (ISP) and reflected in his or her Person-Centered Thinking and Discovery tools.
Host Home
Host home services enable a person to retain or improve skills related to: health; activities of daily living; money management; community mobility; recreation; cooking; shopping; use of community resources; community safety; and to develop other adaptive skills needed to live in the community.
One-Time Transitional Services
OTT services are one-time, non-recurring start-up expenses for persons enrolled in the ID/DD Waiver who are transitioning from an institution or provider-operated living arrangement to a living arrangement in a private residence where the person is directly responsible for his or her own living expenses.
Residential Habilitaiton
Residential habilitation services are supports provided in a home shared by no more than four people. The service assists each person in: acquiring, retaining, and improving self-care, daily living, adaptive, and other skills needed to reside successfully in a shared home within the community, based upon what is important to and for the person, as documented in his or her Individualized Support Plan (ISP) and reflected in his or her Person-Centered Thinking and Discovery tools.
